The Massachusetts League of Community Health Centers (League) was founded in 1972 as one of the country's first state Primary Care Associations (PCAs). Established under the same federal authorizing legislation as the health center program (Section 330 of the Public Health Service Act), PCAs are organized around a set of core functions and competencies that provide a framework for support and assistance to health centers and the communities they serve. The League is a non-profit 501(c)(3) organization registered as a public charity with the Commonwealth's Secretary of State. It maintains a professional staff at its headquarters in Boston and its training center in Worcester.

Position Summary:
Under the supervision of EVP & CSO and in accordance with League policies and procedures, the Senior Program Director is responsible for the management of Connecticut River Valley Farmworker Health Program (CRVFHP) to ensure access to high quality, comprehensive and coordinated health care, outreach, health education and other related services for migratory and seasonal agricultural workers (MSAWs) and their families in the Connecticut River Valley in a linguistically and culturally-sensitive manner. Manage program activities and transactions for accuracy in service transactions and documentation, program compliance, quality assurance and patient satisfaction through data monitoring. Work with CRVFHP staff and Partner Agencies in both Connecticut and Massachusetts directly to reach additional MSAWs, demonstrate quality improvement and strengthen collaborative migrant service organization relationships regionally and nationally.

- Monitor and execute federal grant deliverables and program contracts, including the Request for Proposal application / process, and contracts, and lead grant research, writing and reporting
- In conjunction with the CFO, participate in budget planning to manage annual program budget that supports the provision of outreach and primary care services and all CRVFHP activities
- Monitor and ensure required updates to the annual CRVFHP Request for Proposal application, Contract and CRVFHP Forms and contracting processes
- Complete and submit HRSA required reports, including Budget Period Renewal applications, Uniform Data System reports, Service Area Competition grant applications, etc.
- Monitor data entry in CRVFHP Electronic Medical Records
- Monitor timely and accurate payment of outreach voucher and Explanation of Benefits voucher payment requests
- Streamline and expedite data entry to achieve programmatic goals
- Monitor the design and development of health education curriculum for Outreach teams throughout the year.
- Monitor and ensure development of culturally and linguistically appropriate health promotion materials and e-modules for Outreach Staff
- Be aware of current state requirements for Certified Application Counselors (CACs) and Navigators in Connecticut and Massachusetts; support staff development and knowledge of marketplace
- In conjunction with the CRVFHP-designated Clinical Director and Clinical Department, develop and monitor the annual Quality Improvement Plan, including the QI reporting schedule, and monitor clinical performance measure through the annual Medical Chart Review audit
- Travel within the Connecticut River Valley, including regular visits to access points, care delivery sites and work camps, to meetings elsewhere in Massachusetts and to national and regional conferences; to provide TA, build relationships, and increase program awareness; extended stays in the Valley to provide intense training and oversight to Partner Agencies up to 5-10 times per year
- In collaboration with CRVFHP management staff, plan CRVFHP annual calendar of meetings
- Plan and staff quarterly CRVFHP Advisory Council meetings in the Connecticut River Valley and serve on national and local committees, as necessary to advance the mission of the CRVFHP
- Develop partnerships with other Migrant Voucher Programs, Migrant Service Organizations, National Cooperative Agreements, Agricultural Growers Associations, etc. and coordinate the development of a coalition of migrant service organizations for Connecticut and Massachusetts
- Support CRVFHP staff developing and updating public relations and promotional activities for the program including presentations, website, newsletter publication (Notes from the Field), brochures, posters, etc.
- Oversee the development of technical assistance resources and conduct bilingual focus groups at camps or Partner Agencies
- Actively search for additional development and funding opportunities to support the mission of the CRVFHP
- Work with CRVFHP staff to develop process improvement projects (shared tools, technology, automation, etc.)
- Develop and submit monthly program activity reports (PARs) and pay period timesheets to the League
- Monitor and contribute to preparation of monthly reports for the CRVFHP Board of Directors
- Conduct program evaluation and verification of program activity impact as per guidance and requirements provided by the Workforce Division.
- Report on program evaluation and impact data at team meetings and through the League's association management software.

Competencies/Skills:
- Excellent computer skills - especially Excel (e.g., Outlook, Word, Excel, PowerPoint, web-based databases, online newsletter publication software, online survey tools, online web conferencing software)
- Excellent interpersonal skills
- Experience working with Electronic Medical Records
- Ability to work with minimal supervision
- Ability to work with multiple projects and tasks simultaneously
- Ability to maintain and update organized office records and files
- Effective oral and written communication skills
- Punctual and timely in work assignments and attendance
- Strong leadership, planning, evaluation and problem resolution skills
- Fluency in spoken and written Spanish preferred; bicultural background desirable
- Excellent grant writing skills

- Willingness and flexibility to carry out program goals and objectives
- Ability to work and think independently
- Administrative office and procedures experience required
- Spanish language skills required
- Experience working with agricultural workers a plus but not a requirement
- Knowledge of laws and regulations affecting agricultural workers and other special populations
- Knowledge of federal and state health policy
- Valid driver's license and access to a vehicle
Education/Experience:
- Four years' college or equivalent with three to five years of directly relevant work experience. Advanced degree preferable
- Five to ten years of experience in program management, development, administration, operations or grant management
- Experience working with migrant/seasonal agricultural workers and growers (i.e., labor organization, community outreach and advocacy) is preferable
- Past or current employment or volunteer work with community health centers is preferable

While performing the duties of this job, the employee is required to talk and hear regularly. The employee must be able to remain in a stationary position 80% of the time. The employee needs to move inside and outside the office to access file cabinets, office equipment, and meet with community partners at different sites. The position requires occasionally lifting office products and supplies up to 50 pounds and traveling to various locations for meetings.
